- We would like to refer our patient, Mr James Dolan, into your facility for ophthalmology assessment. He has no significant medical history but smokes 5-10 cigarettes/day.
- On May 12, 2019, he visited our office because of eye pain which had begun 2 days prior to the visit. He had eye redness in only his left eye. As he was diagnosed with acute conjunctivitis, Tobrex 0.3% eye drop q.i.d both eyes, one drop/eye was prescribed.
- On May 14, he visited again with a similar complaint. He experienced swelling and worsened pain in his both eyes. He had experienced a fever and took NSAIDs. His WBC count was elevated (23 x 10^9/L) so Amclavox Duo Forte 875mg/125mg b.i.d for 5 days was prescribed.
- On May 15, the pain had further worsened. He had a watery discharge and dramatically swollen eyelids and periorbital tissue.
