Doctor: | Good morning Mr. Bloomfield. My name’s Dr. Dixon. I’m a local GP standing in for Dr. Wright. What can we do for you? |
Graham: | I’ve been having some problems with my breathing. |
Doctor: | Can you tell me a little bit more about this? |
Graham: | Well, I keep getting breathlessness and wheezing in my chest. It all started about three weeks ago and I’ve been coughing a lot with it – some white phlegm.
I thought it might be a cold coming on. But then after about another week, I started finding it more and more difficult to catch my breath. |
Doctor: | Right. So, you’ve had the wheezing and the breathlessness for roughly three weeks. |
Graham: | Yes. Give or take a day. |
Doctor: | And, you get these bouts of wheezing and shortness of breath every day? |
Graham: | No, they come and go. |
Doctor: | How frequently do you get them? |
Graham: | The first week there was only one I think, and then they started getting worse 3 – 4 times a week. It’s not being able to get my breath. That’s really worrying. |
Doctor: | And so the attacks… Have they increased in the past two weeks? |
Graham: | Yes, they are much more frequent. |
Doctor: | Okay. |
Doctor: | When the attacks come on? |
Graham: | At any time, but they seem to be worse at night and in the morning. |
Doctor: | Have you noticed any change in the severity of the attacks, especially in the morning? |
Graham: | Yeah. |
Doctor: | Do they wake you up at night? |
Graham: | About three times a week. |
Doctor: | I see. You’ve been off work at all? |
Graham: | No, but I nearly didn’t go yesterday. |
Doctor: | Was that the worst so far? |
Graham: | Yeah. |
Doctor: | And have you had anything else with it? |
Graham: | Um… I’ve felt a bit tight across the chest. |
Doctor: | Any pain with it? |
Graham: | No. Just tightness. |
Doctor: | Are you aware of anything that triggers the attacks? |
Graham: | Um… like what? |
Doctor: | Dusts, feathers, new carpets? |
Graham: | No.I can’t really say I am. |
Doctor: | Okay. Have you had any infections recently, like flu, or sore throat, or chest infection? |
Graham: | No, not for a long time except this. |
Doctor: | And what about medications? Are you taking anything? |
Graham: | No. |
Doctor: | No aspirin? |
Graham: | No. |
Doctor: | Are you doing any exercise jogging for instance? |
Graham: | No. |
Doctor: | What about pets? Do you have pets at home? |
Graham: | Um, No, but my neighbors have a cat, but I don’t see it that much. |
Doctor: | Everything is okay at home? |
Graham: | Yeah. Things are fine. |
Doctor: | And what about work? I see you’re a civil servant. Any stress or problems at work, or anything like that? |
Graham: | Mm… Work’s been um…, getting me down recently. |
Doctor: | In what way? |
Graham: | Well, there’s been a lot of changes going on recently and I suppose I’m a bit anxious what with the mortgage in that. |
Doctor: | And this has been getting to you? |
Graham: | Yes. More and more. |
Doctor: | I see. And does the wheezing etcetera continue over the weekends? |
Graham: | Um… No. When I come to think about it, it doesn’t. |
Doctor: | Okay. Some general questions. Have you ever had anything like this before? |
Graham: | No. Never. |
Doctor: | Do you have other illnesses, high blood pressure, diabetes, or heart problems? |
Graham: | No, nothing like that. This is the first time I’ve been ill in my life. |
Doctor: | Has anyone in your family had anything similar? |
Graham: | No, not as far as I know. |
Doctor: | What about eczema? Anyone in your family with that? |
Graham: | Both my sister and my mother have it. |
